It shipped. Now ask them.

Everything up to here was you asking your own team. This is the one stage where the questions go to the people actually living with the sound. Pick the items you want, and you get a questionnaire you can field — twenty-six items, all from the book, all worded for a user rather than a stakeholder.

This is the least charted stage, and that is worth knowing

The book has around seventy questions for the stage before you have any sound, and a handful for the stage after launch. That imbalance is the field's, not an oversight — post-deployment sound research barely exists. Which means the repetition, the fatigue and the switching-off all happen out here, where almost nobody is looking. Treat what follows as a starting point rather than a validated instrument.

Your questionnaire

The items you picked, in order, with a suggested response format for each. Chapter 6 covers how to choose scales and chapter 8 covers running it.
